Select the Enrollment Package from the forms below. Download the PDF to your computer. Open the file in Adobe Acrobat Reader.
How to Fill Out the Form
Once you open the PDF file in Adobe, multiple options will be listed on the right hand side of the menu bar. Select “Fill & Sign” if you prefer to type in your responses. For signatures, you may select the pen icon along the top menu bar and select your font preference.
You also can simply print a hard copy and fill it out by hand and sign the document.
Submit the Application
Email the Filled and Signed electronic copy of the application or a scan the print out of the completed and signed application and email to Compass Academy at compassacademyga@gmail.com
